To my experience every auto catcher including this one will only use the standard red pokeball. It will not use great or ultra balls.

Yes, one single switch that turns on/off the whole device

Q: When you're connected to two separate accounts, how does the voice notification for out of pokeballs or inventory full distinguish between the two?

It comes with a female voice and a male voice.

Q: Does pokemon go need go be open for this to work or can it be minimized in the background?

It still catches and spins with the game minimized running in the background (iPhone). Just need to check it time to time to make sure you are connected. If you are moving too fast, it won't catch anything, but will spin stops if it is close enough even when running in the background.
